Providence Health & Services Behavioral Health Specialist - Primary Care in Portland, Oregon


Providence Medical Group is looking for a Behavioral Health Specialist to join the Collaborative Engagement Team. This position will be for an exempt full time position, Monday - Friday during the day.

As a part of our promise to integrate behavioral health, Providence Medical Group is forming a new department, the Collaborative Engagement Team. This is a great opportunity to identify and engage our high risk primary care patients to provide them with additional support and resources to prevent them from requiring higher levels of care in the future.

In this position, you will:

  • Be responsible for identifying and engaging patients within the primary care population with high risk behavioral health and substance abuse conditions.

  • Collaborate effectively with Primary care treatment team to develop effect care plans that outline treat to target metrics identified from valid measurement tools to assess and document baseline symptom severity.

  • Provide brief behavioral interventions using evidence-based techniques such as behavioral activation, problem-solving treatment, motivational interviewing, or other treatments as appropriate.

  • Participate in regularly scheduled caseload consultation with the psychiatric consultant and communicate resulting treatment recommendations to the patient’s medical provider.


Required qualifications for this position include:

  • Master’s degree in Social Work from an School of Social Work that is accredited or in candidacy status by the Council of Social Work Education (CSWE) -or- Master's degree eligible for licensure as Licensed Professional Counselor (LPC) or Licensed Marriage and Family Therapist (LMFT) in accordance with the Oregon Board of Licensed Professional Counselors

  • Licensed Clinical Social Worker (LCSW) -or- LPC -or- LMFT licensure with the state of Oregon required.

  • 2 years post masters experience in a behavioral health setting.

  • A Certified Alcohol and Drug Counselor (CADC) -or- 60 hours of approved continuing education units (CEUs) in diagnosis and treatment of substance use disorders within 1 year of hire

  • American Heart Association (AHA) Basic Life Support for Health Care Provider completion within 1 month of hire

  • Valid driver’s license and reliable transportation required for travel to clinic locations and off site meetings with care coordinators and/or patients/families

Preferred qualifications for this position include:

  • Additional experience in substance use and/or primary care treatment setting
