Providence is calling a Clinical Pharmacy Anticoagulation Specialist to Providence St.Vincent Medical Center in Portland, OR.

This position will support the expansion of our MS and Hep C service lines. It will support the East and Westside clinics.

In this position you will:

  • Collaborate with providers and caregivers to provide coordinated, comprehensive, high-quality care to patients referred to the Providence Health & Services (PH&S) Anticoagulation and Pharmacotherapy Services for focused drug therapy management of acute and chronic disorders.

  • Be responsible for providing collaborative drug therapy management (CDTM) services in clinic, telephonically, by e-visits, and possibly telehealth.

  • Upon referral asses the patient, initiate and adjust medications, screen for and minimize drug interactions and adverse effects, provide disease/medication education, and explore opportunities to simplify and/or decrease the cost of the medication regimen in collaboration with the referring provider (PCP)/prescriber, patient/their caregiver and other Providence team members.

  • Educate team members to continually advance the service level of practice.

Qualifications:

Required qualifications for this position include:

  • Graduate from an accredited school of pharmacy

  • A post-graduate year residency (PGY1) -or-

  • Minimum 3 years’ experience in area of specialization (ambulatory care with direct, collaborative patient care) in lieu of residency

  • Oregon State licensure as a Pharmacist or ability to obtain license prior to employment

  • Board Certified Pharmacotherapy Specialist (BCPS) -or-

  • Board Certified Ambulatory Care Pharmacist (BCACP) -or-

  • other Board of Pharmacy Specialties certification in area of specialization prior to or within 2 years of hire as appropriate

  • Basic Life Support (BLS) within 6 months of hire

Preferred qualifications for this position include:

  • A PGY-2 residency in ambulatory care
