Providence is calling a Team Coordinator - Happy Valley Immediate Care (1.0 FTE, Days) to Happy Valley Clinic in Happy Valley, OR.

Shift: Monday- Friday 8 Hours

We are seeking a Team Coordinator - Happy Valley Immediate Care who will be a key liaison between patients and their Patient Centered Medical Home Team, including providers, clinic staff, and other health care professionals. Their primary function is to provide leadership for a seamless experience of care for patients calling their health care team, providing answers to patients' questions, or connecting the patient with another team member, tracking clinic quality measures and facilitating outreach to patients regarding services that are due. The Team Coordinator must understand complex health systems, and help patients navigate the system to meet their healthcare needs.

In this position you will have the following responsibilities:

  • Manage daily needs of care team to include: staff schedules, provider Epic schedule optimization (daily access and capacity), and collaboration with CCC to ensure ancillary assignments are completed and patient care needs are met

  • Patient advocate who work closely with all team caregivers to include Clinical Care Coordinator, Providers, Team MA's, Case Managers, Behaviorists, Pharmacist and others to coordinate care for patients

  • Provide training on new team workflows and updated regulations

  • Serve as primary mentor and coach for other caregivers who are answering phones and assisting patients with complex needs

  • Facilitate "one-touch" resolution of phone calls, answering the patients' questions and/or connecting them with a team member for resolution

  • Identify medically urgent or emergent situations and responds appropriately

  • Champion Clinic Quality Improvement projects using established QI principles

  • Additional duties for the rooming position only: With additional training:

  • Greet incoming patients, obtain initial vital signs (Height, Weight, BP, Pulse, Respirations) and in accordance with predefined metrics

  • Document pertinent patient care information

  • Prepare rooms for new patients

  • Stock room supplies and carts

  • Utilize and maintains supplies to minimize waste

Required qualifications for this position includes:

  • Two (2) years of experience working as a Patient Relations Rep in PMG

  • Experience with electronic medical records

  • Strong verbal communication and listening skills

  • Effective leadership skills

  • Effective interpersonal skills

  • Proficiency in Microsoft Windows and other office applications

Preferred qualifications for this position includes:

  • One (1) year of working with a diverse population

  • Knowledge of Lean principles
