We are seeking a Occupational Therapy Assistant (COTA) who will assess needs and provides Certified Occupational Therapy Assistant (COTA) services and direct treatment in consultation with the Occupational Therapist. The COTA functions as a member of the Interdisciplinary Team. Responsible for home care coordination
In this position you will have the following responsibilities:
Function as a member of the inter-disciplinary team, completes Certified Occupational Therapy Assistant (COTA) screen for new or designated geriatric participants. Notify Occupational Therapist (OTR) of need for further evaluation based upon specific criteria.
Provide activity of daily living (ADL) assistance as needed to supplement home care of care givers, based on condition of geriatric participant, home environment and/or abilities of care
Provide COTA, individual and group activities to maintain and enhance the geriatric participants' physical functioning such as in ADLs (bathing, toileting, eating, dressing, personal hygiene, grooming, and behavior
Provide specific individual geriatric COTA treatments as per OTR care plan
Provide appropriate instruction, in-service, assistance and monitoring to other staff as needed, specifically related to ADL care plans.
Participate in committee work related to rehabilitation, program planning, equipment, and other committees of interest to support the operations of the program
Maintain up-to-date treatment plan records, progress notes and other required documentation in the electronic medical record.
Assess need and implements action to get necessary equipment for geriatric participant/caregiver use to enhance and/or support all indicated ADL activities in the center and at home, in conjunction with the occupational therapist. Responsible for maintenance, labeling, cleaning, and inventory of appropriate equipment.
Required qualifications for this position includes:
Graduate of a Certified Occupational Therapy Assistant program.
Current registration and license as a COTA in the State of Oregon.
American Heart Association Basic Life Support (AHA-BLS) for Health Care Provider certification required within 1 months of hire.
Preferred qualifications for this position includes:
- One year (1) experience in the area of Occupational Therapy as a COTA
