Providence is calling a RN Float - Beaverton Nursing Services (0.8 FTE, Days) to Elderplace Beaverton in Hillsboro, OR
We are seeking a Registered Nurse (RN) Float - Beaverton Nursing Services that will provide and supervises professional, independent nursing care for Providence patients that is as evidence-based as possible. Care is based in the nursing process and is consistent with the Oregon Nurse Practice Act. The RN applies knowledge of assessing, planning, ordering, giving, delegating, teaching and supervising professional nursing care delivery that promotes patients' optimal health and independence. All Providence professional nursing practice requires a broad range of general nursing knowledge and expertise. In addition, development and validation of knowledge and skills essential to the assigned department, patient population and/or practice area are crucial. The Float RN provides competent nursing care as a Triage RN, Clinic/Care Manager RN, Community Care/Care Manager RN, and Housing RN. This position may be required to rotate through a weekend shift.The RN participates in and leads clinical teams and may supervise, assign or delegate to licensed practical nurses (LPN's), certified nursing assistants (CNA's) and other health care workers.
In this position you will have the following responsibilities:
Perform comprehensive nursing assessments, in person or by telephone, using appropriate critical thinking skills, nursing theory, research and judgment, to identify the severity, and urgency of the participant care problem(s) and provides advice, guidance and a nursing care plan that reflects participant priorities, in alignment with the Interdisciplinary Team
Communicate and coordinate care processes across the continuum of care in multiple facilities and serves as a clinical liaison to participant caregivers, including family members and staff of the PACE contracted network
Serve as a community care nurse or care manager for a defined panel of PACE participants that includes participants requiring a broad spectrum of care who reside in a variety of environments and may reside in a wide geographic area
Provide mentoring, education, anticipatory guidance and nursing delegation to community caregivers and ElderPlace housing staff in the application and implementation of the nursing care plan appropriate to the caregiver’s scope of practice
Act as a teacher/resource to other team members
Precept nursing students as requested
Collaborate with the IDT in developing and implementing a holistic plan of care appropriate to a community setting
Participate in the development of an interdisciplinary care plan based on assessment and re-assessment when there has been a significant change
Perform procedures and treatments according to accepted clinical program standards
Integrate culturally competent/sensitive participant care
Engage in effective and caring professional communication and behaviors with colleagues, co-workers and interdisciplinary team members
Clinical documentation is timely and reflects adherence to nursing standards, interdisciplinary plan of care and progress toward identified participant outcomes
Perform comprehensive and focused nursing assessments at periodic routine intervals and as needed for changes in ElderPlace housing resident conditions
Required qualifications for this position includes:
Coursework/Training: Relevant training applicable to the position, as specified by the Nurse Manager
Current, unencumbered Oregon RN license
Current BLS required at time of hire. American Heart Association Basic Life Support (AHA-BLS) for Health Care Provider certification required within 6 months of hire
Relevant certifications applicable to the position, as specified by the Nurse Manager
Preferred qualifications for this position includes:
Bachelor's Degree in Nursing
One (1) year acute care RN experience
