We are seeking a Speech Therapist - Beaverton Medical Services who will be responsible for Speech-Language Pathology(SLP) evaluations and implementation of the Speech Therapy plan of care in accordance with PACE policies. The Speech-Language Pathologist works with the Interdisciplinary Team and is responsible to communicate and coordinate with the IDT as well as multiple outside providers, caregivers and family members. The SLP functions independently and may see participants in multiple settings including the PACE center, participant homes, Adult Family Homes, Assisted Living Facilities and nursing homes.
In this position you will have the following responsibilities:
Perform thorough SLP assessments within SLP scope of practice for PACE participants and determines need for SLP intervention, caregiver training and other follow up as indicated.
Develop plan of care in accordance with participant needs, CMS guidelines and IDT approval.
Interact with the public and other employees in a positive manner
Demonstrate an ability to treat a wide range of geriatric diagnoses and be knowledgeable in SLP interventions, goals and contraindications;. Treatment are adapted to the unique PACE population. Request consultation, supervision or continuing education when appropriate.
Provide participant/caregiver education and counseling regarding issues related to SLP treatment, goals and safety risks.
Supervise SLP students, volunteers and other observers as assigned.
Document all SLP assessments and interventions in appropriate agency format and within agency timelines; Document reflect good clinical documentation
Communicate assessment and recommendations to the IDT.
Provide training to PACE staff in appropriate areas related to SLP practice and participant care
Maintain all participant information and records in a professional and confidential manner, per HIPAA regulations.
Required qualifications in this position includes:
Master's Degree from an accredited Speech/Language Pathology Program.
Speech/Language Pathology licensure through the State of Oregon, or eligibility
American Heart Association Basic Life Support (AHA-BLS) for Health Care Provider certification required at time of hire
Valid driver's license
Current auto liability insurance
Preferred qualifications in this position includes:
- One (1) year experience working as an SLP in a geriatric setting
