Providence Health & Services Care Management Supervisor - Community Health Worker in Portland, Oregon

Shift: Monday-Friday 8 AM - 5 PM

We are seeking a Supervisor Community Services - Utilization Review Quality Management that will responsible for daily operations and oversight of the Community Health Workers (CHWs) within the PMG Care Management Department and Providence Medical Group ambulatory care clinics. The CHW team strives to build individual and community wellbeing to underserved populations by providing education and outreach, assist with care coordination and navigation of the health system, and may act as a cultural mediator/advocate, and educator.

In this position you will have the following responsibilities:

  • Monitor the daily activities, including scheduling and daily assignments within the Case Management Program including timely completion of documentation, referrals and other time sensitive work

  • Supervise staff performance and developmental training. Performs annual performance reviews and the development and monitoring of improvement plans according to PHS guidelines

  • Assist in the hiring process, provides orientation and training and works with individual staff on identified performance issues in accordance with PSJH policies and procedures

  • Implement and monitor collection of and analyzes program’s evaluation data and the ability to summarize and communicate key points to Manager

  • Act as a liaison between the department and PMG primary care clinics, specialist offices, and contracted health plans

  • Develop and maintain training materials and workflows for the Community Health Workers

  • Provide support to Manager in the role of working supervisor

  • Promote effective communication with providers, patients, staff, PMG Administration and PHS departments. Develops and maintains positive interpersonal relationships

  • Actively support and help facilitate regular departmental staff meetings to communicate organizational changes and developments

  • Facilitate team development, team function, and problem solving

  • Work closely with Manager of the program to ensure department goals are being met and formulates solutions to meet goals when needed

  • Act as a technical knowledge resource to employees and stakeholders

  • Support staff to identify opportunities for outreach with a focus on building relationships with hard to reach populations through individuals or community organizations

  • Must be willing to meet with staff and patients in the community and/or travel between PMG clinics


Required qualifications for this position includes:

  • Bachelor's Degree in Nursing, Social Work, Health Education or related field

  • High School Diploma or GED 4 years experience as a Community Health Worker or similar Traditional Healthcare Worker and CHW certification may substitute for Bachelor's degree

  • Two (2) years Community health worker experience and/or similar care coordination or social service experience

  • Demonstrated ability to work cooperatively and communicate effectively in an interdisciplinary team

  • Current state of Oregon RN licensure or Licensed Clinical Social Worker License

  • Oregon Community Health Worker Certification at time of hire or within first 3 months of employment

  • Current Oregon or Washington driver's license

Preferred qualifications for this position includes:

  • One (1) year Supervisory experience

  • Bilingual skills
