Providence is calling a Diabetes Educator - Diabetes Education (1.0 FTE, Days) to Providence Office Park in Portland , OR
We are seeking a Diabetes Educator - Diabetes Education that will provide education for clients with diabetes of all ages according to the individualized needs of the client. This process involves a health and education assessment, development, implementation and evaluation of an education plan, taking into account psychosocial, economic, and language barriers. Duties are performed in a classroom or individual outpatient setting in collaboration with a team of RNs & RDs. The candidate accepts professional development and community outreach as an integral part of professional practice.
In this position you will have the following responsibilities:
Actively support and incorporates the mission and core values into daily activities

Demonstrate service excellence and positive interpersonal relations in dealing with others, including clients, families, fellow staff members, managers, referral providers and community stakeholders
Report to work as scheduled and on time per system policy, Time away from work is approved and taken in accordance with system policies
Perform comprehensive client assessment, including medical history, health service access and utilization, risk factors, current diabetes knowledge, skills, attitudes, barriers to self-management with emphasis on socioeconomic, cultural, and linguistics resources
Develop, implement, and evaluate individualized client education plan that reflects the patient's assessment and on-going self-management capabilities
Maintain confidentiality of all information related to patients, medical staff, and as appropriate, other information
Accurately and concisely perform client and program documentation to meet regulatory and reimbursement requirements
Consistently demonstrate and incorporates principles of safety and infection control into daily activities as outlined in Environment of Care, Infection Control and Exposure Control manuals
Maintain knowledge of work-appropriate aspects of Environment of Care program and attends EOC skills training
Participate in regular staff meetings, inservice trainings, and on-going program development
Collaborate with regular staff in a spirit of teamwork to evaluate trends in diabetes education and help the program maintain consistency with national diabetes education standards
Participate in community outreach projects focused on diabetes education according to specific talents and interests
Cross-train in all aspects of diabetes education as appropriate for the integrity of the program
Required qualifications for this position includes:
Registered Nurse -OR- Registered Dietitian; current Oregon licensure.
Three (3) years of experience in treatment and education of diabetes within the last 2 years.
Experience with current adult learning theory in relation to diabetes education.
Experience with the clinical management of diabetes.
Preferred qualifications for this position includes:
Coursework/Training: Formal coursework or credentials in principles of adult learning theory and patient-centered learning
Certification in diabetes education and/or willingness to attain CDCES certification
Experience with diverse ethnic groups
Bilingual in Spanish, Russian, or Vietnamese.
