We are seeking a Director Population Health Solutions - Community Health Division that will supports the Oregon Region in facilitating and guiding this transformation, with a special concern for our most vulnerable populations. In particular, we focus on very low-income individuals, frail and at-risk seniors and communities of color. We will develop and monitor meaningful new population-focused solutions and metrics, collaborate more consistently with community partners, work to understand what our customers truly value and expand caregiver's skills to serve our most complex and vulnerable populations.

The Director of Population Health Solutions will engage a broad group of stakeholders across the enterprise, including Providence Health Plan, to build this foundation and meet the needs of our Providence members. The Director will be responsible for working with Operating departments to set strategy, create and implement major initiatives that deliver more affordable, efficient models of care. This role must balance opportunities to better serve Providence members with internal capabilities and other existing priorities to ensure success. The individual in this role effectively engages internal Providence leaders and teams and external partners in engaging in and supporting transformation priorities. These priorities will include a comprehensive Medicaid strategy across service areas.

This person will also be called on to help develop new partnerships, partner with other regions on population health development, as well as other ad-hoc projects required for the overall growth and success of the population health portfolio. This position in collaboration with Population Health Analytics and Finance partners, will develop key performance indicators to determine program effectiveness.

In this position you will have the following responsibilities:

  • Work to develop annual strategic priorities related to affordability, excellence and member experience and align with System ISFP success metrics

  • Engage stakeholders as appropriate across the continuum to effectively deliver a portfolio of work which improves efficiency, outcomes, experience, and affordability

  • Coordinate with Population Health Council and Regional leadership to develop and maintain a prioritization process for determining deployment of key resources

  • Coordinate and leads the Population Health Council and the Population Health Work Team. Develops agendas, works with members to prepare, and follows up on action items

  • Develop a region wide Medicaid strategy to identify how to most appropriately care for vulnerable populations including social determinants of health

  • Facilitate a region wide CCO workgroup to coordinate Providence participation in multiple, disparate CCOs and to align our priorities and advocacy

  • Partner with care delivery and health plan leadership and caregiver teams in the planning, development, implementation and monitoring of program initiatives that will improve affordability and member experience across the enterprise for Providence members

  • Lead the transition to new value-based business plans and financial reporting tools, focused on membership, cost savings and efficiency

  • Provide analysis and perspective on data related to relevant market trends, industry best practices and regulatory trends in the health care industry

  • Report progress on initiatives within the Population Health team and with the Oregon Executive Council as appropriate

  • Coordinate portfolio of contracts with Providence Health Plan for shared risk populations. Facilitate effective annual cycle with appropriate and timely discussion and decision making

  • Develop key performance indicators to determine initiative effectiveness for ongoing monitoring and notifies stakeholders of problems affecting timelines and resources

  • Coordinate with PSJH System Population Health team and other Regional teams for communicating key priorities, reporting on status of Oregon initiatives and sharing of best practices

  • Work closely with other members of the Population Health team to assure the voice of vulnerable populations (very low income, seniors and communities of color) are represented in Oregon strategic priorities and resource allocation

  • Provide support to local service area leadership teams, as requested, in their development of key population health strategies and development

  • Other duties as assigned


Required qualifications for this position include:

  • Bachelor's Degree health care, clinical field, public health

  • 10 or more years experience leading complex initiatives in a health care or related setting

  • Experience in value based care and population health program development

  • Leadership role in a health care or related setting

Preferred qualifications for this position include:

  • Master's Degree health care, clinical field, public healt or equivalent educ/experience

  • 3 years experience with health insurance plans, funding and administration
