Providence Liaison Clinical (RN) in Portland, Oregon

Providence is calling a Liaison Clinical (RN) - Home Service Liasons (0.0 FTE, On Call Days) to Home Services Building in Portland, OR.

Applicants who meet minimum qualifications will be sent a text to answer a single question to verify you are a current caregiver.

We are seeking a Liaison Clinical Registered Nurse (RN) - who will represent Providence Home Services (HS) at contracted facilities and requires strong communication and interpersonal skills. The role of the Clinical Liaison is to coordinate and arrange home care services between HS and contracted facilities for home health, home infusion, and home medical equipment associated with a clinical referral, except when there is an onsite HME liaison.

The position acts as a clinical resource and educates hospital staff regarding the services offered by HS, and contributes to the patient care plan as it relates to the discharge plan. The Clinical Liaison provides information to ensure a smooth transition for patients and their families following hospitalization.

In this position you will have the following responsibilities:

  • Coordinate care for all services being provided after the ACM/discharge planner provides the patient with freedom of choice for a home services agency and contacts the HS Liaison with an order for Home Services

  • Anticipate supply and equipment needs required to provide home services

  • Meet with the patient to explain home services being provided and obtains necessary documentation to provide care

  • Assess that services ordered are appropriate

  • Document in the hospital record the plan of care for discharge with the name of the agency, phone number, start of care date and services ordered

  • Accept out of area referrals and patient requests for another agency

  • Give referral to Access Services staff to arrange for services

  • Enter patient data and appropriate documentation into the appropriate Home Services databases

  • Coordinate with HS scheduling team to determine start of care date for home services

  • Work with HS office-based Access and Insurance staff, and Infusion Coordinators and Pharmacists to facilitate a smooth discharge for patients

  • Work with the Marketing and Sales Manager, and HS Department Director to update hospital staff on HS programs and contracts

  • Maintain productivity requirements in facilitating timely referrals to Home Services

  • Keep current regarding community health issues and regulations

  • Communicate with others in a warm, helpful, and professional manner while simultaneously building credibility and rapport

  • Give high priority to customer satisfaction and anticipates customer needs

Qualifications:

Required qualifications for this position include:

  • Current Oregon RN license

Preferred qualifications for this position includes:

  • Bachelor's Degree in Nursing

  • One (1) year of full-time experience in home health or hospice

  • One (1) year of year full-time experience in home infusion

  • One (1) year of year full-time experience in discharge planning or care management

  • Experience in home care and in positions that required extensive customer service skills
