We are seeking an Occupational Therapist - Glendoveer Day Center who will be responsible for OT evaluations, and development and implementation of the OT plan of care in accordance with PACE policies. The OT is a member of the Interdisciplinary Team and responsible to communicate and coordinate care with the team as well as multiple outside providers, caregivers and family members. The OT functions independently, working in multiple settings including the PACE center, participant homes, Adult Family Homes, Assistive Living Facilities and nursing homes. The OT assumes responsibility for participants treated by the OTA and OT Aide.
In this position you will have the following responsibilities:
Performs thorough OT assessments, evaluating the physical and functional status as well as cognitive, psychological and perceptual function of PACE participants.
Performs home safety assessments as needed; as well as providing any needed adaptations, DME and caregiver training in home environmentProvide
Develops plan of care in accordance with participant’s needs and IDT approval.
Interacts with the public and other employees in a positive manner.
Demonstrates an ability to treat a wide range of geriatric diagnoses, both psychiatric and physical and be knowledgeable in therapy treatments, goals and contraindications for treatment. Treatments are adapted to the unique PACE population. Requests consultation, supervision or continuing education when appropriate to learn specialized techniques or treatment approaches.
Provides participant/caregiver education and counseling regarding issues related to rehabilitation treatment and goals, safety risks and equipment and home adaptations
Evaluates, recommends and is responsible for obtaining adaptive equipment and DME. Fabricates adaptive equipment as needed. Adapts equipment and techniques for use in participants living situation assuring optimum function and safety. Trains team members and caregivers in the use of adaptive equipment and DME.
Documents all OT evaluations and interventions in appropriate agency format and within agency timelines. Documents reflect good clinical documentation.
Responsible for the instruction and supervision of OTA’s and OT Aides in accordance with the OT WAC’s. Supervises students as assigned.
Participates in Interdisciplinary Team meetings, presenting findings and recommendations.
Participates in other committees as assigned representing the Rehab team
Participates in agency CQI process as assigned.
Provides training to PACE staff in appropriate areas related to OT practice and participant care.
Works independently, managing own caseload and work schedule, including OT treatments, evaluations, staff and caregiver training, home visits, required meetings and documentation
Maintains all participant information and records in a professional and confidential manner, per HIPAA regulations
Required qualifications for this position include:
Bachelor's Degree in Occupational Therapy
Valid Driver's License
Current auto liability insurance
Current licensure as an Occupational Therapist by the state of Oregon
American Heart Association Basic Life Support (AHA-BLS) for Health Care Provider certification at time of hire
Preferred qualifications for this position includes:
Master's Degree in Occupational Therapy or OTD (Occupational Therapy Doctorage)
1 year experience working as an occupational therapist in a geriatric setting
