Clinical Pharmacist - Credena Health - Home Services Specialty Pharmacy at Home Services Building

Clinical Pharmacist - Credena Health - Home Services Specialty Pharmacy that will be responsible for coordination of medication related services as it pertains to specialty pharmacy. Responsibilities include promotion of evidence-based, safe, effective drug therapy, standardization of medication practices across the continuum, and coordination with drug procurement, inventory management and billing processes.

Credena Health pharmacists interpret, dispense and monitor all physician prescriptions. Pharmacists provide the best possible drug therapy to Credena Health patients under the highest of professional standards.

The pharmacist works closely with nurses, physicians and allied healthcare professionals to assure that patients receive quality pharmaceutical care, delivered in a safe and efficient manner.

The Credena Health pharmacist performs all duties in a manner that promotes a team concept and reflects the Providence mission, core values, service excellence and behavior standards.


Required qualifications:

  • Currently licensed as a registered pharmacist in good standing by the Oregon Board of Pharmacy or the ability to become licensed prior to employment

  • Comprehensive clinical pharmacy knowledge and practice experience base with emphasis in the field of specialty pharmacy

  • Able to evaluate and synthesize data and apply evidence-based medicine principles to practice

  • Demonstrated ability to effectively establish and maintain working relationships with peers and constituents at all levels of the organization

  • Excellent interpersonal communication skills and the ability to communicate effectively in verbal and written form with the health care team and with patients

  • Minimum of two (2) years experience in hospital IV pharmacy practice
