Providence is calling a Physical Therapist - Gresham Day Center (1.0 FTE, Days) to Providence Elderplace Oregon in Portland, OR.
We are seeking a Physical Therapist - Gresham Day Center who will be responsible for PT evaluations and development and implementation of the PT plan of care in accordance with PACE policies. The PT is a member of the Interdisciplinary Team and responsible to communicate and coordinate care with the team as well as multiple outside providers, caregivers and family members. The PT functions independently, working in multiple settings including the PACE center, participant homes, Adult Family Homes, Assistive Living Facilities and nursing homes. The PT assumes responsibility for participants treated by the PTA and PT aide.
In this position you will have the following responsibilities:
Performs thorough PT assessments evaluating physical and functional mobility status of PACE participants including but not limited to ROM, strength, balance, sensation, coordination and gait. Determines need for PT interventions, caregiver training and durable medical equipment.
Develops plan of care in accordance with participant needs and IDT approval.
Interacts with the public and other employees in a positive manner
Demonstrates an ability to treat a wide range of geriatric diagnoses and be knowledgeable in therapy treatments, goals and contraindications for treatments. Treatments are adapted to the unique PACE population. Requests consultation, supervision or continuing education when appropriate to learn specialized techniques or treatment approaches.
Provides participant/caregiver education and counseling regarding issues related to rehabilitation treatment and goals, safety risks and equipment needs.
Documents all PT evaluations and interventions in appropriate agency format and within agency timelines. Documents reflect good clinical documentation.
Responsible for instruction and supervision of PTA’s and PT Aides in accordance with PT WACs. Supervises students as assigned.
Participates in Interdisciplinary Team meetings, presenting findings and recommendations.
Participates in other committees as assigned
Participates in agency CQI process as assigned.
Provides training to PACE staff in appropriate areas related to PT practice and participant care
Works independently managing own caseload and work schedule including PT treatments, evaluations, staff and caregiver training, home visits, required meetings and documentation.
Maintains all participant information and records in a professional and confidential manner, per HIPAA regulations
Required qualifications for this position include:
Bachelor's Degree in Physical Therapy
Valid Driver's License
Current auto liability insurance
Licensed or eligible for PT licensure in the state of Oregon.
American Heart Association Basic Life Support (AHA-BLS) for Health Care Provider certification required at time of hire.
Preferred qualifications for this position include:
Master's Degree in Physical Therapy or DPT
1 year working with a geriatric population
