We are seeking a Registered Nurse (RN) Community Care - DEPARTMENT that will provide and supervise professional, independent nursing care for Providence patients. Care is based in the nursing process and is consistent with the Oregon Nurse Practice Act. Applies knowledge of assessing, planning, ordering, giving, delegating, teaching and supervising professional nursing care delivery that promotes patients' optimal health and independence. All Providence professional nursing practice requires a broad range of general nursing knowledge and expertise. In addition, development and validation of knowledge and skills essential to the assigned department, patient population and/or practice area are crucial. This position may be required to rotate through a weekend shift.
The RN participates in and leads clinical teams and may supervise, assign or delegate to licensed practical nurses (LPN's), certified nursing assistants (CNA's) and other health care workers.
In this position you will have the following responsibilities:
Serve as a community care nurse or care manager for a defined panel of PACE participants that include participants requiring a broad spectrum of care who reside in a variety of environments and may reside in a wide geographic area
Apply appropriate nursing theory and research to clinical practice
Perform comprehensive nursing assessment, consistently using appropriate clinical thinking skills and nursing judgment to develop, refine and evaluate an evidence-based nursing plan of care that reflect participant priorities, as part of the interdisciplinary plan of care, that is appropriate to a community setting and across the continuum of care
Perform and delegate procedures and treatments according to accepted clinical program standards
Diagnostic, Monitoring, and Intervention Function: Assessment and ongoing monitoring of patient condition, initiation of appropriate nursing and medical response, and documentation
Protect vulnerable patients from complications of the clinical care environment
Perform a comprehensive nursing assessment
Consistently use clinical judgment and take action appropriate to the patient situation
Utilize the clinical systems and available resources to develop, refine, and evaluate an evidence-based nursing plan of care, as well as, communicate and coordinate care processes
Utilize clinical systems to input data that will allow all nurses to "tell the patients story"
Anticipate the expected outcome of therapies/interventions and plan care appropriately
Helping: Attention to the individual person and family as patient, listening to the patient’s story and adapting care accordingly
Management of patient comfort including pain management, skin care, spiritual and comfort care measures
Tailor caring processes to the individuality and uniqueness of each patient and family
Advocate for his/her patient’s rights, needs, and desires
Integrate culturally competent/sensitive patient care into the patient and family’s plan of care
Engage in effective and caring professional communication and behaviors with colleagues, co-workers, and interdisciplinary teams
Teaching/Coaching: Teaching and coaching patients and their families as well as other healthcare providers
Consistently perform a comprehensive patient teaching/learning assessment
Implement and comprehensively evaluate a patient/family teaching/learning plan of care
Act as a teacher/resource to other team members
Collaborate with other disciplines in establishing an education plan
Monitoring and Ensuring the Quality of Health Care Practices: Accountability for safe practice, identifying and correcting problems, participating in quality improvement activities and employing evidence-based practice
Consistently focus on improving nursing care quality by readjusting care strategies and priorities
Participate in quality improvement and nursing research activities at the unit level
Demonstrate nursing practice that support enhanced patient outcomes, including the National Patient Safety Goals, nurse sensitive outcomes, and other strategic quality initiatives
Utilize statistical reports and evidence-based tools to support decision-making
Integrate the framework of "data to wisdom" into the practice of nursing
Organizational and Professional Development: Skill in working in, communicating with, and leading teams
Be accountable for personal behavior and ongoing professional behavior and development
Remain current with required credentialing (e.g. licensure, BLS, ACLS, PALS, etc.), unit-based education, performance verification activities and competencies, and informatics competencies
Support the education process for those new to the Nursing profession, those nurses continuing their education or expanding their practice and other team members
Maintain his/her practice within the legal parameters of the Nurse Practice Act, the Nursing Code of Ethics as set forth by the American Nurses Association, and the Nursing Social Policy statement
Maintain patient confidentiality and data security
Use effective communication and interpersonal skills to be an effective team member
Effective Management of Rapidly Changing Situations: Skill in managing rapidly changing situations including the ability to assess, make decisions, and orchestrate a team response
Initiate an appropriate emergency response (RRT, Code blue, 911, color codes, etc.)
Effectively alter the nursing plan of care as new data and situations emerge
Appropriately respond to unanticipated and adverse effects of nursing and medical interventions
Coordinate, order, and meet multiple patients’ needs by realigning priorities as neede
Appropriately alert other health team members to pertinent changes in patient status
Qualifications:
Required qualifications for this position includes:
Coursework/Training: Relevant training applicable to the position, as specified by the Nurse Manager
Current, unencumbered Oregon RN license
Current Basic Life Support (BLS) at time of hire
American Heart Association Basic Life Support (AHA-BLS) for Health Care Provider certification within six (6) months of hire
Relevant certifications applicable to the position, as specified by the Nurse Manager
