Providence is calling a Care Management Resource Coordinator - Care Management (0.0 FTE, Days) to Providence Portland Medical Center in Portland, OR. This is a Per-Diem position to cover open shifts within this department.

We are seeking a Care Management Resource Coordinator - Care Management that will provide a variety of support functions for the patients and Care Management Department. Coordinates and manages all written and oral communication inter/intra-departmentally.
In this position you will have the following responsibilities:
At the direction of Care Management team (RN-Care Manager, Social Worker, Care Transition Coordinator) assists with the following duties:
Assist with chart documentation
Assist with patient discharge transportation
Assist with Skilled Nursing Facility authorization, insurance verification and benefit eligibility
Gather patient information from medical record and fax referrals to appropriate sources: SNF, ICF, Acute Hospital, AFH, ALF, RCP and appropriate community resources
Complete data entry on PAF tracking tools and vouchers
Process check requests to pay invoices for the PAF account
Meet with patients and family as directed by RNCM
Attend and participates in interdisciplinary rounds as indicated
Information, data and technology support:
Design, coordinate, and manage basic databases to store information related to Nursing Informatics and Nursing Quality as requested
Assist care management team with data collection and information activities as requested
Ensure confidentiality and security of protected health information when reporting data and information by eliminating key identifiers from data presentation or using alternative methods to identify patient specific information
Participate in committees and projects that my include both facility-based committees as well as statewide committees as requested
Act as an IS Liaison
Submit and monitor IS requests on an ongoing basis
Run and provides monthly reports to care management manager and to staff as needed
General Department Duties:
Provide meeting support, prepares support documents, reserves rooms, services requests, take meeting minutes
Order and maintain office equipment, supplies
Develop and maintain filing system to include correspondence, forms, related documents to care management department
Communication & interpersonal relationships:
Serve as the initial contact person and receptionist for Care Management
Provide initial level of response to questions from hospital staff, patients and community members
Screen and direct persons and telephone calls coming into Care Management appropriately
Coordinate and schedule interviews and appointments for staff on request
Convey a warm, personal, caring attitude by phone and in person
Maintain confidentiality of all information
Qualifications:
Required qualifications for this position includes:
High School Diploma or GED
Three (3) years of Administrative experience preferably in healthcare industry
Evidence of written and oral communication skills
Preferred qualifications for this position include:
- Associate's Degree
