Providence Health & Services RN Case Management Supervisor in Portland, Oregon
Providence is calling an RN Case Management Supervisor (1.0 FTE, Days) to Providence Office Park Portland in Portland, OR.
We are seeking a Case Management Supervisor that will be responsible for the supervision of the daily operations of the Case Management Department, specifically the work of the Clinical Case Managers and Clinical Support Coordinators. This work includes coordination of the Case Management Program, staff supervision and liaison between the department and PMG primary care clinics, specialist offices and contracted health plans.
In this position you will have the following responsibilities:
Monitor the daily activities, including scheduling and daily assignments within the Case Management Program. Implement improvement activities. Work in conjunction with the department manager to provide trained and knowledgeable staff to resource the program
Supervise staff performance and developmental training including ongoing performance assessment and the development and monitoring of improvement plans according to PHS guidelines. Perform annual performance evaluations for the Case Managers and Clinical Support Coordinators. Recommend hiring and performance management
Assist in the hiring process, provide orientation and training and works with individual staff on identified performance issues
Assist with the annual performance reviews for the case management staff, under the direction of the department manager
Act as a liaison between the department and PMG primary care clinics, specialist offices, and contracted health plans
Develop and maintain training materials and workflows for the Case Managers and Clinical Support Coordinators
Provide support to Manager in the role of working supervisor
Promote effective communication with providers, patients, staff, PMG Administration and PHS departments. Develop and maintain positive interpersonal relationships
Actively support and help facilitate regular departmental staff meetings to communicate organizational changes and developments
Facilitate team development, team function, and problem solving
Required qualifications for this position include:
Graduation from an accredited school of nursing
3 years Clinical experience
Demonstrated ability to work cooperatively in a multi-disciplinary team
Reliable personal transportation
Current state of Oregon RN licensure
Current Oregon or Washington driver's license
Current recognition by a national professional organization in case management at time of hire or obtained within 2 years of hire date
Preferred qualifications for this position include:
Bachelor’s Degree on Nursing, or related field
Home health, skilled nursing facility and/or community health nursing
1 year Supervisory experience
Washington RN license
