Providence is calling a RN Coordinator-Clinical Neuro Oncology (1.0 FTE, Day) to St Vincent East Pavillion in Portland, OR

We are seeking a RN Coordinator-Clinical Neuro Oncology - that will works closely with the Neuro-Oncology Providers & Social Worker. There are several key functions in the management of patients with Central Nervous System

In this position you will have the following responsibilities:

  • Provide direct patient care in collaboration with Neuro-Oncology Team and multidisciplinary specialists according to professional nursing standards of care and staff policies/procedures & clinical guidelines to include

  • Telephone triage - Identify clinical problems, psychological and social needs - provide advice, recommendations and appropriate treatment options. Seizures, cognitive changes, behavioral changes, treatment administration questions, & treatment effects (nausea, fatigue, constipation, skin problems, pneumonia), are some examples of common calls

  • Physical assessment

  • Monitor laboratory results and notify patient/caregiver of change in treatment plans per Neuro-Oncology provider guidance

  • Monitor, advise and follow up on adjustment of medications and side effects

  • Documentation in electronic medical record

  • Promote health maintenance and illness/injury prevention behavior for Neuro-Oncology patients and caregivers

  • Order entry medical and consumable equipment

  • Play a vital role in helping improve quality of life (QoL) by educating the patient on the management and control of symptoms and offering support following diagnosis:

  • identify patient and caregiver needs.

  • Assess current educational/support resources.

  • Plan patient interventions, which may include creating patient education handouts in collaboration with Neuro-Oncology providers, individual education or developing a support group with social worker

  • Develop patient education materials at a low literacy level.

  • Evaluate impact of interventions.

  • Identify coping and adjustment strategies, motivation and support mechanisms. Empower the patient and the family to identify and reach realistic goals, throughout the course of the disease

  • Promote health maintenance and illness/injury prevention behavior for neuro-oncology patients and caregivers

  • Multidisciplinary Care Coordination:

  • Work closely with the multidisciplinary team members and coordinates multidisciplinary services. Optimizes referral coordination to ensure patients receive consults timely

  • May assist with Brain Tumor Board conference coordination

  • Act as a liaison between the patient, different specialists, hospital and community services to meet the needs of both patient and caregiver.

  • Recognition of transitions of care needs & coordinates with Palliative Care, Home/Health & Hospice providers

  • Collaborate with ancillary programs to develop or enhance services to support the patient and caregiver.

  • Participate in Clinical Program Services Brain Tumor Focus Group calls and network with other Providence Neuro-Oncology Centers.

  • Collaborate with ancillary programs to develop or enhance services to support the patient and caregiver.

  • Triage & facilitate brain tumor second opinion consults to appropriate provider

Qualifications:

Required qualifications for this position includes:

  • Bachelor's Degree in Nursing

  • Five (5) years Nursing experience which includes experience working with patients with neurological or oncologic diagnoses & their families, and 1 year experience in an ambulatory setting, preferably in a medical office or clinic setting

  • AHA BLS certification

  • Current Oregon RN license

Preferred qualifications for this position includes:

  • Experience with seizure patients
