We are seeking a Supervisor Clinical Hospice - Hospice Access who will supervise skilled and non-skilled care to terminally ill patients and families in their place of residence, hospital or other facility, and upholds Service Excellence Standards of Behavior.

In this position, you will:
Responsible for the oversight of day-to-day operations of the Hospice Access Services department which includes referral/intake, clinical triage and admit teams.
Act as a liaison to hospitals, clinics and other primary customers regarding Hospice Access Services operations and ease of access for Hospice services.
Ensures the continuing high quality delivery of hospice services
Provide staff mentoring, orientation, and training, troubleshooting, coaching and counseling, hiring/firing, and performance appraisals.
Complete Key Performance Indicator (KPI) tracking for intake and referrals, trending of data, and reporting as required.
Maintain staff quality performance accountability and productivity.
Conduct interdisciplinary patient care reviews with clinical staff on a regularly scheduled basis.
Coordinate and participate in necessary updates and communication across hospice clinical teams.
Develop and maintain updated documentation, reference information, and systems for referral, intake and triage teams.
Assist in resolving systems, workflow, and documentation problems. Monitor care according to Quality Assurance standards and Utilization Review processes, participating in the provision of in-service education as needs are identified.
Required qualifications for this position includes:
Current Oregon RN Licensure
Current BLS (Basic Life Support for Health Care Provider) -OR- ACLS (Advanced Cardiac Life Support) within thirty (30) days of hire
Two (2) years of clinical experience in Home Care -OR- Hospice with experience in long term care
Valid driver's license
Driving record consistent with GOP Safe Driving Policy 330 Incident Point Matrix at time of hire
Preferred qualifications for this position include:
Bachelor's Degree in Nursing
Two (2) years of experience in management, preferably in Hospice or Home Care
Community based hospice experience
